Welcome to the Athletics Parent Portal

In an effort to streamline athletic paperwork, LyfordCISD is now offering online forms. The beginning of each school year, it is mandatory that all student-athletes and parents fill out the appropriate LyfordCISD and UIL paperwork before they will be allowed to practice or compete in athletics. All other required paperwork will be done electronically. The parent/guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of the primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically.

Parents, you must fill out each form:
Remember that to submit online appropriately, you and your child must fill out and sign in all required spaces. The athletic staff is not responsible for any physicals lost or not turned into the athletic trainer in person or e-mailed to the athletic trainer directly.

1. To access the online forms hold your cursor over the “Parent/student Electronic fill out Participation Forms” tab. Then, select Texas under the STATE dropdown menu. Scroll down and click on LYFORD. You will be directed to the LyfordCISD School Rank One Sport Page.

2. Click on start online forms, you can begin filling out information or you can click on REGISTER NOW and create an account. We strongly encourage you to create an account. Having a registered account allows you to update any information throughout the school year.

3. To begin, click on the form name and fill out the information requested. The Parent/Guardian must have their student present along with the student’s birth date and ID number to complete the forms. If you do not know the answer to a question, please answer the question with N/A, or NONE.

4. To sign the document, click inside the signature box and hold your mouse down; this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an I Pad, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus

5. Once you have filled out and read the information on each page, make sure you hit the SUBMIT button at the bottom of the page.

6. The hard copy of your Pre-participation - Physical Examination Form must be turned in to your student’s campus athletic department. All other forms will be submitted electronically.

8. The Parent-Student-Athlete Handbook can be printed for your reference on the Lyfordcisd web page. The handbook includes important UIL Health and Safety information as well as guidelines for athletic participation. There will be a signature page within the Electronic Participation Forms stating that you have been provided and agree to follow all rules and guidelines within the handbook
